Uh, the restaurant i worked at which was fine dining just had the waiter's put the tips into the computer. The manager doesn't have time to add in 120 tips in a night.
I don't care if it's the manager or not. Point is, the cash tip was pocketed and then a tip was illegally added since none was apparently left by the OP.
Assuming the OP's memory is right, which it probably is, then of course something happened which raised the total amount. Checks could have been swapped, the wrong card could have been tipped, the $5 cash could have disappeared and the waitress might have felt wronged or the company might have some shady policy.
This is all a moot point. It doesn't matter how it happened. OP needs to call the restaurant, explain the situation and will probably get a fix. If not, avoid that restaurant and file a credit card dispute.
