Once, the chap I worked with actually extracted the wrong tooth! Wasn't the end of the earth, the patient didn't really need the tooth and I think it had extensive caries down to the pulp, but nevertheless, a mistake was made. He took the correct tooth out and actually saved the patient £35 as he didn't charge them for the other tooth
Secondly, we were doing some serious work. Twin extraction of the front two teeth with false teeth immediately put in place over the wound (wound heals around false teeth: looks a million times better), so lots of work needed to be done. The dentist was sitting over the patient (ie patient's head was kind of above his crotch, dentist was looking down) with his mask down. In a moment of deep concentration a long string of drool came out the dentist's mouth and nearly fell on the patient. Luckily he brushed it away with about 3 inches to spare before the patient got a wet suprise (haad their eyes closed!).
