Did you know you can get a papercut from cardboard? I was a Teacher's Assistant and I was cutting up boxes, then bam, big gash on my finger. Owwwwwww. If you really want to explore further this would require knowing what kind of signal the brain receives and its meaning when there is a problem. Then just scale it extremely high and make it come from everywhere. To produce this physically you would have to tear the body into pieces (Jesus wept scene from Hellraiser?). The subject still has the experience and reaction without primary physical harm but those kinds of signals certainly would not come without cost.
Basically it would be like hacking into a large complex and triggering every sensor they have to indicate a problem. What kind of nightmare would that be?
