What's the most painful thing you can think of?

Seriously, in my own personal experience:
Severe Arthritis in practically every joint in your body.
Bone spurs on the spine so massive they are actually rubbing together.

I can think of a lot of very painful things, but most of those will heal or stop hurting eventually.
Arthritis never stops.
 
I'd imagine it would be some form of poison that inflicts maximum agony possible whilst keeping you alive and conscious. Perhaps combine it with physical torture.
 
The treatment to relieve priapism. Might not be the absolute most painful thing in the world, but it's up there.

do you speak from experience? Can't be THAT bad.

I would say getting pierced all over your body 500 times tying 15 foot rope to each peircing, dropping you into a 20 foot pit filled with razorblades and concentrated habanero pepper juice.
 
seeing loved one cooked and tortured alive, and you're next..and at the same time, you have kidney stones on both kidneys, and appendicidis.

Having holes drilled into the pulp cavity of your teeth and electrodes connected up there to the electricity, shocking you, whilst a wire-wool pipe cleaner attached to a slow-spinning drill was forced down your urethra.
