You can get a referral to a dermatologist if they can't figure it out.

I'd get that checked out by a doctor, it could be a infection of some sort or worse. You don't want to let it get out of hand.

Probably eczema or psoriasis. I'm thinking psoriasis since you mentioned occasional itching. Get it checked. While not as bad as other stuff like cancer it can be a pain to deal with and if you catch it fast enough they may be able to do something. I have it all over my head and back, it sucks.
