Because that requires mroe measurment and even then how you measure and what type of tool you use can give different numbers.

Because it's a body MASS index. Regardless if it's muscle or fat, it's still addional mass. The more mass you have the harder your heart has to work to pump blood to where it needs to go. It's still a risk factor.
