Ok, seriously though, what you have is a ganglion cyst on that testicle. Sometimes known as a Bible bump, the common treatment is to hit the offending cyst with a Bible or other large book.

I had a similar issue this past summer. A fair amount of pain, a lot of swelling of one testicle. There were two real possibilities given the symptoms: testicular torsion, which involves a blocked blood supply to one of the testicles and can lead to a dead testicle, or epididymitis, which is an infection of part of the plumbing on the back part of the testicle. This happened to me on a weekend, and my doctor told me to go to the ER to make sure it wasn't the torsion thing. They had to do an ultrasound to distinguish between the two, luckily it was the infection in my case.

hmmmm, that's interesting. If it's an infection, I wonder if my kidney surgery had anything to do with it.
 
10 years ago I woke up to my left nut swollen to the size of a very large tennis ball, with some pain and serious dizziness.

My wife went nuts (!) looking at it and would not stop crying (she always imagines the worst right at the outset). Of course this would happen on a saturday morning, so I had to go to urgent care, where one intern had no clue what to make of it. Luckily a real doc agreed to step in and gave me anti-biotics, and said it was because of 'fluid retention', though he could not say what kind unless he did some biopsy or something. But the anti-biotic worked and I never went back.

I did have some good days after that - I spun the 'fluid retention' phrase to my wife as, do you want to help me out with my medical condition and make sure I don't retain any fluid down there?...😉 Of course, she was also mighty relieved after those few hours of anxious waiting...

So you might be lucky and this might just ... blow off ...

If your wife's inspections were so rare, I'm surprised that you hadn't taken over inspection duties.
 
If you come back with anything, blame your wife for not inspecting it more often. Preventative maintenance is key.

After a rare "inspection" by my wife of my testicles a couple days ago, she commented that my left testicle seemed to be much larger than my right.

Lo and behold, after a self-inspection, it's about twice the size. Can't seem to recall when the size difference first started, but I do know I have had some discomfort in that area for awhile. I always chalked it up to my underwear or pants being snug.

A year ago I had a thorough examination as part of my kidney donation. I came back with flying colors, however, I don't recall them examining my testicles.

A rudementary google search indicates 1) it's either a form of cancer or testicular torsion, and 2) if there's no pain, it's most likely cancer.

What are the chances I have the cancer? Going to see the doctor Monday morning. I hope I don't die 🙁
I had a similar scare about a decade ago, mine weren't even close to symmetrical to begin with, but I discovered a roundish lump in the upper part of the (larger) left one. I went in to my regular doc, she looked, she was stumped, she had two more (female) doctors come in and look, they left and a male doctor came in and looked. I was alone about thirty minutes and they probably were all looking at textbooks for at least part of that time. Eventually she came back and said she was pretty sure it was a spematocele. She told me to take NSAIDS and ejaculate as much as possible for a few weeks and maybe I could clear it that way. I also got a referral to a urologist who confirmed that it was a spermatocele, and that it could go away on its own or he'd be happy to slice it out, but that choosing not to worry about it was also a safe option. He used a Doppler device to show me that it was liquid filled and not solid tissue which I found reassuring. Its still there, doesn't hurt, everything works, and the ladies don't seem to notice.
If you got the cancer, don't hesitate to get it cut out. You can get a fake one in its place if you're worried about the aesthetics. I hope its nothing, good luck.
