He didn't lose the weight. At the point he's at in the picture, you cannot lose the weight. You don't have enough muscle mass to move that much weight enough to burn enough calories to start losing weight. It requires surgery.

Your post makes it sound like you maintain weight without eating. He has to be eating an assload to support his frame already, sure hes sedentary but losing weight ONLY requires eating less than your body burns. And every pound requires some amount of calories to maintain

Not really. It's input versus output, yes. Even at 1000lbs, he's most likely bed-ridden, which means he doesn't burn much energy each day at all. He could probably eat very little and still maintain his weight. Changing what you eat and how much is only half the picture. If he's not burning much energy, it doesn't matter how little he eats.
Fat people don't have to eat constantly to maintain their weight.
