Maybe he just really likes the firefox icon ;)
It's pretty spiffy if I say so myself, I have the same thing, a shortcut, dock, quicklaunch and start menu for firefox.

Or make sure you are always near clicking an icon you want.

I just reinstalled windows completely and the dock and all, have still to clean up all the Firefox icons, anyway they're gone now.

edit: lots of really cool desktops in here. It's been a few years since I was into customizing mine. Now it's just the plain old windows (even less than xp's default style!) and a mess of icons for me
