I'd bring it up to her first, don't make your pussy excuses about how she won't heed it. Bring it up, see what happens, if she continues to do it bring it up again and then go and see your boss. Big deal if she knows it was you. I reported my old company to the health department after I addressed a smoking issue with management.

You already told her, now it is time to go over her head after ignoring you. If she has a problem with it, tell her that her priorities are screwed up.

she didn't ask. I told her that even though I was new and didn't want to step on any toes I had seen one of the staff members texting while driving the van, to the point it would be veering in the lane on the road. I asked if she could put out a memo or something along those lines asking staff not to. She was concerned, and thanked me for bringing it to her attention.
 
There's got to be a license plate on the van for it to be on the road legally, right? PM that to whoever will call it in...
