That's how it typically works at stores like Home Depot, but at the grocery stores around here that have them (Weis in particular) there's a belt with a sensor that checks each item as it passes under. When the bagging area gets too full, it tells you to bag some of your items. I didn't have to screw it up more than once to realize that if you scan an item before the previous item passes under the sensor, it makes you rescan both items.
Edit: (The ones at the Giant I used to go to on the way home from work had it the way you describe)
I use self checkout lanes all the time, rarely have a problem.
