I think people would hold off, because of MAD. They know if they launch against the US they are going to get flattened as well. As long as Russia and the US don't fire at eachother, it would be a very limited exchange. Not every country has nuclear weapons. Mainly, USA, Russia, England, France, China (India, Pakistan, Israel, N Korea)
If an exchange happened between the US and N Korea, The only people capable of retailating against the US mainland would be Russia, France and England aren't going to get involved in the mess.
The problems will arrise if the US uses a weapon on N. Korea, Pakistan or India will see it as a sign that it's acciptible to use nuclear weapons in war and someone will launch and there will be retailations. Once it starts, Israel, may decide it's time to take out the trouble Arabs and launch against them. I don't think any of the "Super power" nations, USA, France, Russia, England, or China would launch against another because they all know that they are going to get fucked in the ass by a Jackhammer in Oklahoma.
