I'm a delivery driver, and imo it's rarely the drivers fault when orders are switched. At the place I work, the kitchen is responsible for tagging the pizzas / sides and making sure the order #s match with the order, and throwing in extra sauce, dressing, etc. We have to trust that the kitchen did their job ... we hardly ever have time to double check the pizzas. If there's ever a mixup, we always bring a fresh pizza and let the customer keep the old one ... and I personally never expect a tip, but of course it's nice when they throw me a couple bucks for driving back out. On the other hand I'd rather they not tip me again if it were my fault. The worst is bringing a 2nd free order to someone cause another driver or take-out screwed up, and getting no tip
