The former is what is recommended. You are encouraged to wear a mask if you have symptoms of an illness but given an outbreak it's good practice to wear a mask even if you don't have symptoms. Two reasons being some diseases are communicable before symptoms appear and the second is that even if you don't have the virus you want to minimize your contact with your mouth and nose in case you do pick up the virus on your hands. I have no idea why this is so hard for you.

With the population densities in the US, wearing a surgical mask to prevent the spreading of an illness is an oddity. In areas where you have population densities in the tens of thousands per square kilometer, wearing a mask because an appropriate necessity.

How is it not appropriate? It prevents saliva and mucus from being placed upon common surfaces by trapping sneezes and coughs and by preventing contact with the mouth and nose by hand. It prevents your hands from transmitting viruses off of common surfaces by preventing them from coming into contact with your mouth and nose.

mostly it doesn't protect you from air born transmission which people worry about. people wear the other mask type because they are clueless mostly. in asian countries its sometimes worn out of courtesy when ill so as not to spread germs, but thats a different matter.
