There are multiple reasons to get adult circumcisions.
This article is incredibly vague on details. Unfortunately there are multiple reasons why a partial penectomy may be necessary.

For instance, there are terrible infections called http://en.wikipedia.org/wiki/Fournier_gangrene , that can occur, especially in diabetics.

There are also Penile Cancers (interestingly, the gardasil vaccine protects against penile cancer, so vaccinate your children, both male and female).

I don't know what the cause, but step one in any surgical case that you need to do something different is to immediately discuss with the family or surrogate unless absolutely emergent.

Only a very small subset of phimosis is caused by lack of hygiene. This has been definitively shown in research.

Just FYI, you need to seriously fuck some shit up, like fucking unbelievably badly, to have to amputate a penis during a circ for a "surgical error" to necessitate that severe of an option. The dick is surprisingly nimble and able to heal. I mean think of the 10+ years of abuse at your own hands you put on it during your teen years.

This is not the whole story. This guy either had Infectious Gangrene, Necrosis, or Suspicion of Cancer to necessitate a partial penectomy.
My guess would be necrosis. Phimosis and Paraphimosis, if left untreated can quickly necrose and become non-viable.

Google necrotic paraphimosis if you don't believe me/never want to sleep sound again.
I, in the last month, took a dude for emergent circumcision for paraphimosis. Another 2-3 hours it would have been penis chopping time.

As I understand it, if a dude's hygiene down there isn't that great, infections can get in to the foreskin, and circumcision is considered to be "the way to go" in that scenario (presumably because the condition is chronic at that point).

Thanks, you got me looking at my penis to see if anything is wrong with it. I don't even know what to look for. There's some discoloration on my head, is this bad? LOL

So glad my parents had me circ'd as a baby, I don't think they were told about those possibilities (b. 1959) but grateful anyway.

We should chop bits off because there's a small chance the bits that we don't chop off might need medical attention later.

didnt the guy meet with the surgeon before the surgery to go over the procedure? I though was standard practice. everytime i have gone under the knife or my wife the surgeon ALWAYS has come into the room a hour or so prior to the operation.

Every time you went under your wife the surgeon got a consult? Didn't that ever strike you as unnecessary?
 
Not sure why the outrage. Half the human population seems to be doing just fine without that particular appendage, granted, they're a little on the crazy side. (Actually, downright insane would be a better description.)
