No issue has been found except one floor mat problem.

I never knew it wasn't even a Toyota floor mat that caused that.That floor mat was taken from a different model, so it didn't fit right in the first place. What if someone had taken the spare tire out and put that in the driver's footwell? Would they issue a recall for that as well?
And either way, shifting into neutral would have prevented that crash along with most of these other crashes, regardless of the extent of the problem.

So despite this starting months ago is there a single case of unintended accident-causing acceleration that is clearly the failure of a Toyota car?
I believe it was a Lexus RX-series all-weather floor mat in an ES350 that caused the issue.
Toyota took a shotgun approach to the floor mat issue - they advised people to stop using the floor mats, issued a recall to make the accelerator pedal smaller and less likely to stick, and installed a software update that included a brake override (so if you hit the gas and the brakes at once it ignores the gas).
It was irresponsible to use the wrong floor mat, but I wouldn't be surprised if a lot of people did it. Reuse the all-weather mats from one car in another one because of the cost. Seems like a cheap-ass thing to do, especially in a Lexus. But on the other hand, the other factors (a pedal that is apparently more prone to getting stuck and the lack of a brake override) also contributed to the crash.
I took a shot gun approach and declined all the recalls when the dealership took my car into service. Refused the floor mat since it was just retarded. Refused the brake override since that's also retarded. My pedal was not recalled.
It was absolutely hilarious since everyone at the time was jamming the dealerships with service appointments for the recalls, and I was the one who went in and declined all of them.
