The old medical from my wife's union covered it @ 100%. I had it done on the 2nd to the last day before the new contract went in to effect, and the new medical didn't cover it.. My wife was too scurred. So she still has her contacts (sucker!). This was a year ago September.
The process was painless, albeit wierd IMHO.
I apparently was an anomaly, as my eyes "hurt" like fvck a few hours later. They tell you to just go to sleep afterwards, which I tried to do. But based on the appointment time, I wasn't the least bit tired. They say it feels like sand in your eyes. I say it felt like a whole damn beach.
I finally did fall asleep, then woke up around 3 in the am cuz our daughter was crying. I was like, "OMGWTFBBQ! I can see!!!"
