Ideally you should always approach from their 6 o'clock. Preferably from an elevated position, with a jumping or pouncing motion, and be sure to announce your intentions with a menacing growl.

If a horse's head is facing away from you, do not approach. If you approach and they turn their head away, cease approach immediately. If possible do a barrel roll.
Depends on the horse. The one I had growing up way my best friend, his name was Buddy, he never once kicked me, though a different one once kicked me in the head. That'll ring your bell.

Now I could take a cheap shot here, say something to the effect of how that 'explains a few things.' But don't worry, I won't.

Hopefully it was a small, old horse, sans shoes. My dad's half Belgian, half Clydesdale mare once spooked at something and stomped my foot real quick, when I was 12. Didn't work out too well.
