I've seen this before...when there are several accidents at an intersection/onramp/offramp they reevaluate the design and change it. The fact they let the bridge sit there without extra lights/warnings/etc should be getting people in government positions fired.

It's practically indestructible because it is a railroad overpass (which usually means a/the freight company owns it), so they can't do anything about the bridge. It's been there as long as the city has been there, but it all just grew up around it. IIRC, it's pretty close to the DBAP and so trendy downtown Durham, so it is a high-traffic area. ...
If anything, I think they can lower the road under it, but there is no moving that track. They should also work on re-routing or just preventing certain vehicles from approaching that intersection.
They are in the process of raising the bridge now, that's how i found that compilationThey need to put multiple padded hanging / swinging foam rollers farther back and along all the side streets approaching it. Not a bar that could snag vehicles and payloads (some vehicles on those side streets might be turning away from the bridge anyway), but a large lightweight foam roller thing. Any interaction with it should set off loud horns, blasts of water mist, flashing lights, and giant flashing signs: "DO NOT PROCEED [LEFT/RIGHT/AHEAD]. YOU WILL IMPACT OVERPASS!"
