How to correct this sentence?

How about this?

"One surviving artifact, known as the Emerald Tablet of Hermes, was discovered in his tomb by Alexander the Great."

I added the word "artifact" to make it sound a little less awkward although I don't know the context this sentence is in.

Who does "his" refer to?

I changed your's a little, but for now it is this unless someone can come up with something better:
Alexander the Great is said to have discovered a surviving artifact known as the Emerald Tablet of Hermes within the tomb of Hermes (Cockren 2).

One that did survive is said to have been discovered in his tomb by Alexander the Great is known as the Emerald Tablet of Hermes (Cockren 2).

Active voice:
The Emerald Tablet of Hermes, one that did survive, was reportedly discovered in his tomb by Alexander the Great (Cockren 2).

Most of the other suggestions have nothing to do with changing the sentence from passive to active voice.
