How far up from the floor is a towel bar supposed to be?

Depends on if you're a manlet or not. I think 5' would be a good level, but the world makes everything for the lowest common denominator, and makes my back hurt :^(
 
High enough up that you don't have to stoop and not so high that you have to reach. Want us to get a research team from MIT to run the numbers for you?
 
There's really no one right answer...48" is standard for MOST towel bars, but if the bathroom is for kids or someone in a wheel chair...36" is appropriate...IIRC, 16" for mounting above a countertop or toilet tank.

There has already been plenty of research done by MIT on this topic.

The scientific consensus is that the proper height of a towel bar, from top of finished floor to center, is .7164 Smoots.

What if you're using extra-long towels, like 4.5K Faradays or larger?

i've used one of those, it's horrible. the towel is too long and drapes all over the toilet, disgusting. And it's actually 24" above.

if you're thinking about putting it over the toilet, don't. but if you absolutely must, 30" MINIMUM

That would be insane. Absolute bananas.

A towel bar height of .7164 Smoots allows for hanging towels that fall within the middle 95th percentile of all towels and is just right for normal Americans. If you're Canadian, and have access to running water, this applies for hanging your bath pelts too.

Any length above or below 2 standard deviations must either be placed on a shelf or pulled through a towel ring.

I suppose it depends on the bathroom layout, but 16" IS standard for towel bars/ towel rings over a vanity. Those aren't meant for bath towels...but rather guest/hand towels...over the toilet...unless you have very limited space, that might also be an appropriate place for a guest/hand towel...and, yeah, 24" might be more appropriate for over the tank...
 
High enough so the towels don't touch the ground, as that is against national towel rules. While not exactly a law, it's still patriotic to follow them.

Should also be at least 6' away from the toilet, this is based on research by the International Dental Federation (IDF) which found that toothbrushes stored closer become contaminated by airborne fecal particles resulting from the flush.

I don't recall how I determined the height of mine when I installed them, I think I just kinda eye balled it based on the towels I planned to put on it.
