I was atop a stairwell in BC2 when I took out a fellow assault class on the opposing ruskie side. Then from out of no where a medic came running up the stairs soon after him...so I started shooting! Somehow, just as I dropped the medic he managed to revive his fallen comrade and by the sheer grace of god the once fallen assault ruski happened to awaken facing me and shooting.

I died falling forward down the stairs 🙁
 
In BFBC2 died to a hacker that was spamming his assault rifle and hit me literally half way across the map. I said something in chat, the admin checked it out and banned him. Dedicated servers ftw

1) Protection from Magic (Ribald, for around 1k) or Protection from Undead and Carsomyr.
2) Berserk + whatever +5 weapon you can get at the time.

My main character's a berserker, and I have a paladin who just got Carsomyr. I dropped four traps where kangaxx spawns, to get rid of his "lich" phase, then when he went demi, I berserked on my main char and used a protection from undead on my paladin (the only person with a +5 melee weapon... actually probably the best weapon, as it dispels on hit) and whacked away. Kangaxx kept trying to imprison my main char (as he was closest), but the berserk prevented it from happening. My paladin whacked away with about three 2-3 damage hits every round (oil of speed), and eventually he kicked the bucket. No casualties! yay!

I usually don't like tactics that use above a certain amount of cheese, but I made an exception for Kangaxx, as he throws out imprisonments like free candy, and those... well, they're like King cheese.

Crushed by the first Ogre in DA:O on Nightmare. My mage got aggro and ran out of mana, running in circles trying to lose aggro, stomped and knocked me down. Grabbed me up and CRUNCH my life was gone. Alistair and the other two chumps were soon to follow.
 
Got shot in the head with an AWP during a scrim when I was trying to relocate.

I got shot to death by a soldier inside an army base in STALKER: SOC "Complete Mod" 2009

That's ok though, because I reloaded and got my revenge in full.
 
Fucking Final Fantasy X-2.

Went deep into the ass of the cloisters and got jumped by an Elder Drake. Fucker got three one-hit kills before I did anything. And to top it off I lost about half an hour of work and assloads of AP's. I hate the long ass fucking grinds in Final Fantasy, though I admit I like the Job system and was happy to see it return. Its cool to customize your party exactly how you want it, especially when certain items and abilities let you use other abilities you've mastered.

Also, last night in Act of War, my little bastard Jefferson got killed. He's tough, but in AOW an explosion that hits a small building is almost guaranteed to kill the infantry in it.
Automatic end game. I hate having fragile heroes in any RTS, they're way more trouble than they're worth.
 
Mount & Blade Warband. Wasn't paying attention when I decided to try to help hold off a siege.

Turns out, I was helping 50 guys versus 1000. Sadly, this was not Sparta.
 
Fallout 2, Radscorpions were trying to steel my Brahmin, all I had was a spear and my fists. I killed them but the poison finished me off.

I got knifed in a house near flag B on White Pass (BC2 Conquest). I didn't notice him hiding in the attic; he dropped down behind me and knifed me in the ass.
 
Final Fantasty. I'm in the volcano level and see a chest, I go to open it and am ambushed by a red dragon. It casts Blaze and kills all of my party except my fighter. I kill him and get the Flame Mail armor but had no more potions or any way to heal so I died to a fire lizard in the next battle...
 
Yesterday afternoon in L4D 2. Think I was playing survivor, was last one standing after my team got jockey/tank-raped. Being holder of the grenade launcher is quite useful when you're swarmed.
