You almost certainly will need some type of business permit and a health inspection (sanitation).

Keep in mind that whatever permits, approvals, etc. you fail to obtain, your competitors will point out to the authorities.

If I get the cert, can I do direct deliveries from my home? Not sure but I read that you can't do that. Am I wrong?

You negotiated a good lease and location I'm assuming? I'd like to know the details of this adventure you're taking, both the good and bad. Start a blog maybe?

Retail is such high risk. Delivering to your consumers from a food truck sounds like a good idea and lower risk.

We need more drama in AT, I'd say he should start this business solely based on our advice and see what happens.
 
Is getting incorporated and insured enough? Do I need permits?

Why not seriously consider getting a job with someone who already sells smoothies (or similar) locally ?

Then after a period of time (such as a year or two), you will be in a much better (experienced) position, to consider doing this, yourself.
