Usually both, because I usually don't wear a belt or shirt that I have to tuck in.
eh?
I almost religiously only unzip, regardless of what I'm wearing, as in whether I have any shirt tucked in or not.
If the shirt is kind of long, I simply unzip, and then pull up the shirt with my free hand, and take care of business.
But now that I think of it, maybe this also depends on boxers/briefs?
Wearing boxers only, it's super fast just to undo zipper, and then undo boxer opening.

If you're like one of my roommates, you just don't bother zipping up in the first place.
Unzip FTW.
PS
This reminds me of that quote from Babylon 5: "Okay. It's morning. You're getting ready to go to work. You pull on your pants. Do you fasten and then zip or zip and then fasten?"
I always button up first and then zip up.
Oh, and button-flys are a bitch! I think I've owned a single pair in my life, and while I can usually do with just undoing a single button or two, far more work than I'd prefer to do.
