There are supplements out there that contain all three, I've used them, and they are usually a bit more expensive, but from what I can tell, there really is not much difference between the ones that contain all three and the ones that only contain two out of three. But don't take my word for it, because everybody is different.

I don't know what your goals are, but from the reading I've done, I think the most important factor is to choose Glucosamine sulfate since it is more effective than the cheaper alternative, Glucosamine HCL.

Here are two I found on amazon.com which include all three ingredients, including Glucosamine Sulfate and not Glucosamine HCL:

Actually, I don't believe glucosamine sulfate is more effective than glucosamine HCl. Research isn't really good enough on glucosamine in general to say which is better, but I believe the required dose is actually less in glucosamine HCl if I'm not mistaken.

Overall, the research on glucosamine is not even close to conclusive. It can assist with discomfort related to osteoarthritis, but it usually doesn't make changes that are visible on imaging (xray, MRI). Chondroitin and MSM don't really have any research on being beneficial on their own or making glucosamine more beneficial.

Why are you looking into taking this? Is it as a preventative measure? Do you have a joint pathology right now?

Pricewise it seens to be the chondroitin part that is most expensive. I get glucosamine with MSM for about the same price as plain old glucosamine. I've used HCL and sulfate, and I personally don't notice the difference, but after a month of taking it (for example) - I do notice the difference if I stay off for 3-4 days.

Source someone with chondromalacia who runs alot.
