Just a question for OP, after connecting the cable did you make sure to enable the DisplayPort 1.2 channel in your monitor since you haven't mentioned it ?

Guys, I finally got it working.

It turns out the Generic PNP driver was being used by Windows. After doing a fresh install of Win 10 I still was having issues, but as soon as I loaded up the LG driver it hummed along beautifully.

Thank you all for your input! I learned a lot from y'all. 😎

As a side note: My new Acer 144Hz monitor came in today, but I was getting a horrible flickering out of the box when I set it above 60Hz. After a lot of troubleshooting, I found out this is an issue a lot of RX 480 owners are having. Now I'm REALLY debating returning everything and going full Nvidia.

There was a post about a new driver that seems to address the issue of flickering in the desktop for Free-sync monitors. Might wanna give it a whirl?

As someone who just joined the adaptive v-sync club - it's amazing and hopefully you can get your setup working as expected.
