I just don't know what people expect on April's fools. Anything too obvious and it would have been a waste. Anything that was a good skoorbing got flames. Some people just don't know what they want.

Ask around. You'll find that most people know someone who has a serious medical ailment -- or who has died from one. People who experience that stuff don't usually find it good subject matter for jokes.
