Fallout 3 definitely qualifies as "getaway into another reality". Especially in my case, since I live in the DC area, it's interesting to explore an alternate reality version of DC. The devs did a good job as far as attention to details when they were creating the game world and environment.
Gameplay-wise, I'm still kind of undecided as far as whether I like the game or not. It's heavy on the exploration, so if you're into non-stop frag-fests like the UT games or Diablo clones, it might not be your thing.

GTA IV is more of an open game, you'll have to keep yourself busy most of the time, where-as fallout 3 has you going on quests. Both are probably great games, but depending on your taste one might be better for you. I'm sure neither will really dissapoint though.
