A straight answer on this from a typical suburban town:

First, 95% or so of people tip. For a $1.50 delivery fee the average and median tip is $2 easy.. I'd say 60% give $2. 30% give $3, and the rest give something else (mostly zeroes, $1, and a few high tippers).

The driver is usually compensated for gas only, around $1. Maintenance is not covered so the average drive is costing the driver about $1. So with a $1.50 delivery fee and adequate service, you should be tipping $2 minimum (in effect you're giving a $1 tip to the man).

In other situations use your own discretion. Free delivery you should pay a delivery charge anyway as that charge goes for the gas mostly, so $3 minimum (in effect a $1 tip). Some chains have started charging insane delivery fees upwards of $4, giving drivers only $1 or so of that. In this case fucking over the driver is understandable as the driver should find new employment and stop this practice, however in my opinion it is best simply not to order from those establishments.

I'm surprised that so many people don't tip, I usually only give $1-$2 mainly because I already paid the delivery charge... I try to just go pick up my pizzas to avoid the whole tip thing, but many of the places don't allow it after 10 or 11.

The only time I ever see a "service charge" in a restaurant that I sit down at is when there is a large party. I'm sure they have that just so that you don't stiff the server, who has less tables to manage overall because of the larger party. If the service was good, I'd consider tipping over the service charge, dependent upon the amount of the charge.

Yes they do. They have to be better at their job. That's like saying someone at a high end firm doing the same job as someone as a small firm doesn't deserve more pay. There are higher demands, because in high-end restaurants the customers expect great service. The idea of tipping at expensive restaurants is basically to be more critical of performance when judging tip percentage.
