Yes, the insurance industry derives it's rates largely from risk factors. But have you never heard of a sales commission? If Amex opens up it's offices and pays salespeople to tool around and cold call people that raises the premiums they must charge. Assuming Costco provides them with referrals and human interaction is kept to a minimum the online order could save Amex a lot of money, even after the discount and the referral fee to Costco.
