Well I wouldn't go that far because BP is a tough little ah heck. Why we don't commonly get it is due to our ancestors having resistance which allowed them to reach maturity and breed, passing on their genetic resistance. But like any other microbial disease, the act of treatment also provides the same benefit to the pathogen. Some survive the "plague" of antibiotics and pass on that quality to future generations and that leads to antibiotic resistance. A relative change in the mechanism of virulence then makes it far easier to pass on and potentially impossible to treat. Our only advantage then is sanitary precautions as you point out, but the plague once again becomes a killer, even in reduced numbers.

I don't worry about that very much but Ebola and a few other diseases scare the crap out of those familiar with it. We don't even know how it spreads. We have some clues but the actual mechanism? Beats the hell out of us. There's also the possibility that Ebola could become infective by airborne particulate. Think "Andromeda strain" on steroids.
