both. the thread on the screw is jacked.
Who was the last person to screw it in? Only an incompetent person would try to tighten a screw at risk of cross-threading. If in doubt, the thread needs to be cleaned with a tap.

i have no clue how the hell it happened but it did, and it's resolved now. the screw and t-nut are done for.

Why did you even bother to bring this up? The difference between a screw and a bolt, in terms of linguistics, is overarchingly trivial as the word is almost never used on its own, without context, to describe a fastener.
