Voodoo2 was the first card (Good old STB Blackmagic before they bought 3dfx) and I used included sound cards until I got the Hercules Game Theater XP.

Soundcard: Adlib (it played only midis and still way before soundblaster 1 came out) ISA based for my 386 sx 16
Videocard: Voodoo 3 2000AGP for my AMD k6-3 400. The computers before had all standard 2mb memory etc..

Soundblaster Pro, I think? First videocard was integrated, I'm pretty sure, but first one bought was probably Voodoo2.

Hmm.

First video card: some generic Trident ISA VGA card
Sound card: ISA SoundBlaster (packaged with the 1x cd-rom drive ;))

I believe Creative Labs SoundBlaster Pro and Maxtor Mystique 2MB ($200) so I could play Tomb Raider in 3D acceleration. Worked pretty good, though it didn't have even simple bilinear filtering, so there were squares everywhere. Still loved that game.

Whatever was in a Compaq Presario 4112. That damn thing could actually run Quake II fairly well.

I don't remember the sound and video card that came with my first computer when I was starting college. I just remember that the video card came with a grand total of 4MB RAM :p

The first two parts that I actually bought *after* I got that computer were a Creative Labs RIVA TNT and an SB-Live! soundcard.
