Replace lost title
To replace a title:
1. Complete and notarize an Affidavit of Loss form. If there is a lien holder on the title, the lien holder must apply for the duplicate title.
2. Submit the completed form:
* by mail with a check or money order for $9 to:

OR
* in person at any vehicle licensing office with cash or a check for $9 plus service fees.
Your duplicate title will be mailed to you in 8-10 weeks.

Affidavit of Loss/Release of Interest
The Affidavit of Loss/Release of Interest may be used two ways:
To replace a lost, stolen, or destroyed title
Use this form to replace your Washington Certificate of Ownership (title) if it has been lost, stolen, or destroyed.
Note: You do not need a replacement title to release interest in a vehicle or vessel you are selling, trading, or giving away to a Washington resident.
To apply for a new title
You may use a notarized Release of Interest form instead of a signed Certificate of Ownership when applying for a new title for a vehicle or vessel.
All legal and registered owners shown on the title must sign the Release of Interest to transfer the title to the new owner. If the form is signed in the presence of a licensing agent, the agent can certify the signatures and you do not need to have the form notarized.
