I'm thinking more along the lines of lingering stink, or filth that might not be apparent with a cursory should-i-buy-it inspection.

Well I'd probably replace the mattress. Although a decent mattress can get a little pricey, especially when compared to buying the entire rig for $250.
The bed frame itself looks pretty nice though.
