Even that question is meaningless as one places extra hot would be considered medium somewhere else. I do my sauce about 2 parts Franks to 1 part butter, then a dash of extra cayenne pepper and a splash of vinegar. Comes out hot, but not painful.

Buffalo kinda sucks. Plenty of "hot"/wing sauces out there that have a much better flavor than buffalo. Some places I go to, even their vague named "hot" flavor is tasty.
