Windows XP system boot disk

I have a friend whose hard drive is having issues. I'm trying to make a boot disk for him to boot to and run chkdsk to possibly fix the problem. I tried testing it out on my computer, but when I try to boot to the floppy is says something like "I/O Error, Press any key to continue". I do so and it says the same thing again. It's been a while since I've made a floppy boot disk so I may be doing it incorrectly. I just formatted the disk as a "MS-DOS startup disk". Can someone please refresh my memory?

BTW, he does not have a Windows XP install CD, which is why I'm trying to make him a floppy boot disk.

What you need in your tool kitty is a Ultimate Boot CD for Windows CD. Floppies always give you problems specially if you dig out an five years + floppy that had been gathering dust somewhere. Go to UBCD4Win.com and create a UBCD4win cd, unfortunately you have to make one, with minor reconfiguring and updating, using a legitimate XP cd, and use it to run various diagnostics including chkdsk, memtest etc.. You will find that hour's time well spent and rewarded. Please be aware it may be a violation of msft's eula to use this cd on another pc, especially one where the owner can't find the original XP cd. But I do this all the time.
