unless she changed her name to her maiden upon the death of the spouse. she was a Mrs when the spouse died and she is still Mrs, unless she changes it or remarries.

yeah, old school formal would be "Mrs. Frank Jones" for a widow, slightly more "modern" formal would be just "Mrs. Susan Jones"
