How has this not been done in a reasonable manner? Why can I not buy a live stream direct from NBC for say, $9.99/month?

Some 3mbps (H264) stream would be a goldmine for them compared to the costs. Are they afraid of pissing off their content providers? They would still get ad revenue for it.
 
They are afraid of losing the billions of dollars they get from cable providers. Your $10/ month doesn't even make them consider changing.

In Canada, the main television networks (CTV, Global, CityTV) which are equivalent to the main US networks receive no money from cable networks.

Are you sure that NBC et al actually receive money for their main channel? Ignore SyFy and digital speciality channels.

Networks are added to cable systems mostly for free, some have tried to hijack Time Warner, Cox, Comcast for $$ claiming the cable co's. are benefiting from free content which they are, but if your selling advertising on your network your gonna have a hard time telling those paying for air time that you just forced a major cable co. from rebroadcasting your signal thus eliminating millions of homes from seeing the ad your paying for..

It's free almost as soon as its over on bit-torrent and free on hulu shortly there after.

I don't even watch my cable TV. I prefer to watch em on my own terms and no ads. Bit-torrent away my friends.

Actually it is the backlash from the local stations they fear. This is why DirecTV/DISH are forced to waste tons of satellite space carrying scores of duplicate stations all for the privilege of you being able to see all those ultra cheesy local commercials for car dealers, carpet cleaners, and pay day loan providers.
