I think they've all been coined by different journalists. Apparently "Bond villans" sell newspapers (people don't like to read past the name!)

Well, considering that her face looks like a puffy vacuum cleaner, I assumed she was good at something. Perhaps Clinton will confirm it in his memoirs.
