Which of these are correct uses of "then"?

I ate, and then I flossed.
I ate then flossed.
I ate and then flossed.
I ate. Then I flossed.
I ate. Then, I flossed.

All okay? I'm mostly never sure whether there should be an "and" before it or not.

they are all fine. you don't need the comma in the last one (typical rule is comma after a prepositional phrase of 3 words or more) but it doesn't make it grammatically incorrect.

I'd say the bottom four are acceptable.

Typically you should not put a comma before the word "and". It is a common mistake. The main exception is when you have a list of things such as in this sentence:

I like red, blue, and green.

I realize the comma before the "and" in a list is debatable. However there are many cases where it is required to understand the sentence. Thus I perfer always using it to avoid possible confusion.

In the last sentence, the comma is acceptable but it is being phased out of written text as it is not necessary. In other words, hdeck was correct.

Yes you are correct. Putting a comma before and takes place of the semi-colon for combining two sentences.

Eh? Both are fine, but you don't put a semicolon in front of "and."
