Generally, you install either the plastic or metal standoffs. If your case came with metal standoffs and your motherboard came with plastic standoffs, then you probably just have extra.

There also are motherboards which are larger and need more standoffs (for example, EATX vs. microATX). So again, the standoffs are probably extra.

Snap a plastic standoff in the board where access to a threaded fastener is not possible, for example, where the board might go behind a drive cage. Most of the time, use of plastic standoffs is not necessary if correct assembly techniques and good quality cases are used.

I've seen motherboards in OEM systems where a single plastic standoff had been intended for the bottom-corner mounting hole nearest the case rear in a conventional ATX orientation.

I"ve also seen cases which provide a brass-equivalent to the plastic standoff -- large (3/8" dia), slotted brass fittings which slide into irregularly shaped holes so that they are secure to the motherboard pan (of the case) which pushed into the narrower end of the hole. They have threaded fittings which -- in that orientation -- fit the standard ATX motherboard hole. These had been intended for the upper-rear motherboard mount -- perhaps one or two others.

Be CAREFUL choosing which brass standoffs you use. The height of the appropriate standoff (i.e., 6.5 mm, etc.) is determined by the overall design of the motherboard. the usual standoffs are about 1/4" high; others -- 3/8" or 1/2". Your PCI/PCI-E boards will not match the case-design with the wrong standoffs.
