where do you register a business name?

My mom is starting a business and she asked me to find out where you get a form to register the name of her business.

Depends on if she is starting her biz as a Corporation or as a Sole Proprietorship:

-For a Corp u need to Register the biz name with the State, then The County Clerk's Office, and then the City u're doing biz in
-For a Sole Propritorship u need to register your biz with The County Clerk's Office and then the City u're doing biz in
