yup. When i bought mine from radio shack it had the fan attached to a small 486 heatsink, and had a 4-pin power connector. i just unscrewed the 4 self-tapping screws, and took the fan off, then used the same screws and put it on my voodoo3 2000PCI
are you planning on overclocking it or something? because if you aren't, unless the excess heat is causing other components trouble, i'd just leave it as is, and save the money and blocked slot next to the voodoo

if it fries, they'll replace it for you under warantee
edit: the one i got from Radio Shack was called a "computer CPU Cooling Fan for 80486 Processor"
btw, o-cing a voodoo3 2000 doesn't give you too much of a boost, but for some games it can be worth it. i got mine to 175mhz
