When you ship stuff to Canada...

I'm getting a package ready to be shipped to Canada. I printed the shipping label with paypal, but it gave me five versions of the same form, with only one marked with the postage. They have little numbers on them.
1-Customs Declaration
2-Customs Declaration
3-Dispatch Note
4-Post Office Copy
5-Sender's Copy

Where do all of these go? 1 is the only form to have the actual postage shown. Also, do I have to sign all of them at the bottom and do they have to have a mailing office date stamp? I've got the little customs envelope, but need some help. Thanks guys!
