I say nothing at all. The door is locked. People always knock and then immediately go for the handle so the question has already been answered for them.

Its better than being in mid-dump and having to say "occupied" to a complete stranger. You make a noise, they figure out that someone is in the stall, and that's the end of that small predicament.
