When I drink milk on an empty stomach it makes my guts hurt

weird, but it depends on how much you are drinking, at which speed, and at which temperature it is.

If you're guzzling down a glass of cold milk then no wonders.
 
If the milk is cold, you are getting cramps - kind of like a brain freeze when drinking a slushy drink too fast, but in your "guts".

If the milk is warm/room temp, you have AIDS.

Dairy is kind of hard on the stomach, on an empty stomach it will just amplify that.

Had this HUGE ice cream cake once, I was not hungry for an actual meal so just got a desert at a restaurant. Sore stomach for like 3 days. I'm not even lactose intolerant, but apparently I do have my limits. I'd have that cake again though, it was awesome.
