Everyone at the table is done?

This is a somewhat of a debate. Some see it as rude to take plates away before everyone is done and some see it as bad service if plates are not taken away as soon as that person is done eating.

I don't mind either way as long as the waitor doesn't snatch the plate away as I'm putting the last bite in my mouth. That sort of annoys me.

Yeah, as long as I'm finished, I really don't care at all. There are way more important things in my life than worrying about when someone takes my plate.

Individually is nice because you don't accumulate a stockpile of dirty plates in the center of the table. But if the restaurant is busy I don't expect the waitress/waiter to be that on-the-ball with it. Generally, I don't mind. Just be speedy with drink refills and I'm happy.

Pretty much. Honestly, I don't think I could even tell you what normally happens at restaurants I go to. All I know is that at some point after I'm done eating, someone comes for my plate.

Depends on the restraunt/setting too.

Romantic dinner at a nice place, they better wait.
Night of wings and ribs at a steakhouse, take it away to make room for more.
