What's the worst you have ever @!*$ed up at work?

Biggest mess up money wise I have seen was at the bank I work at. My coworker got do to the budget for our computer department. Well couple of weeks after audit comes in and tell us that we have MS office 97 pro installed and no licenses for pro. We had licenses for standard but not pro, whoops! We asked them if we could reinstall to standard but they said hell no. We had to buy all the Pro licenses $150,000. The coworker was very up set even crying a bit. Her boss was pretty nice and they reworked the budget. She kept her job but it was the only time I saw her really screw up. You don't fuck with budgets...

Runs the financial department of a medium sized company I believe. Actually Keith is it a medium or large company?

Medium-large I guess. Not large for the US, that's for sure, but relatively decent-sized for up here.
