About an year ago, I was standing in line at the bank counter, when I saw this guy, wearing a black jacket, burly guy, getting impatient about standing in line...

Suddenly he takes out a gun and waves it around and everyone goes silent. I tried to duck out of his way but he thought i was lunging at him and fired at me. Thank goodness that bullet went through my shoulder without any serious damage. But that one second when I realised that he was gonna shoot at me, wow, that was terrifying. Just a second after he shot me, the guy from behind the shooter stood up and knocked him out. Then I blacked out.

I thought this was going to be one of those stories that ends with the intro to Fresh Prince.

Face down on a dirty pizzeria floor with a gun pressed to the back of my head.

For me it was when I had my accident. I still don't remember crashing or the ride in the ambulance. The scary part was waking up with a doctor looming over me asking me if I knew where I was. I guessed hospital. He took my neck brace off and asked if my neck hurt. Nope. He asked what did hurt. I started doing an inventory:
Head: hurt like hell (13 stitches in my forehead and eyebrow)
Neck: not bad
Shoulder: hurt really bad
Chest: burned like a bitch (My chest crushed the steering wheel)
Stomach: seatbelt left a bruise
Legs: hurt from hitting the underdash
The best for last: My peener was burning and I couldn't figure out why! Turns out they put a catheter in. (I guess when they asked for a urine sample, I told everyone to leave the room so I could pee in private. As soon as everyone left I lit up a smoke in the emergency room)

I had no recollection of what had happened. They told me I was in a serious car crash. I asked if I hurt anyone and no one knew. I was scared shitless until I found out that I had only hit a telephone pole.

Pulling up to my basic training unit, packed into the back of a cattle truck, seeing the line up of Drill Instructors just waiting for us... that plus the 2 hours that followed.

Then 4 months later road marching back from our final FTX around 2AM, and pulling security on top of a fire ants nest during one of the rest breaks. Body swelled up, throat closed shut, couldn't breathe, thought for sure I was going to meet my maker. Was rushed to the hospital via ambulance and spent the rest of the night too afraid to fall asleep because a DS would come in and yell at me for sleeping when I shouldn't be =P

Breaking my neck in a swimming pool and floating to the top face down completely unable to move anything but twitch my shoulders. I was just about to drown when folks realized I was hurt...I seriously thought I was a goner.

One night my husband had a seizure. I thought he was going to die. Hands down scariest thing I have ever experienced.

I dove into a pool and hit my head on the bottom very hard once. I temporarily couldn't move and floated to the top, too. I regained my movement pretty quickly, but DAMN that was scary.

Blacked out from ruptured spleen (felt something wrong, didn't know what) while driving... thankfully I made a turn onto a small street just before I blacked out completely. Woke up (seemingly minutes later) from the act of actually pissing myself and found myself in the middle of the road with the handbrake pulled. If I hadn't awoken, I would've bled out. Got a friend to drive me to the hospital (and blacked out again while getting out of the car). They re-hydrated me while I shook uncontrollably, blood transfusion, spleenectomy, 6 days in hospital and found out just how much you depend on your stomach muscles for the slightest body movement. Now I'm spleenless and have a nice 6 inch vertical scar on my stomach to show off.

A couple years ago we took this big family vacation to Europe and part of it included skiing on this glacier near Innsbruck. I'm not a complete skiing noob, but that glacier was just a little bit out of my league, especially since I decided to make "just one more run" before going to the bottom and resting. I had some cheap goggles and snow was blowing all over the place; I could see the lodge some 400 yards down, but I couldn't see the moguls about 200 yards down. So, I hit these moguls at what seemed like 20 miles an hour, on a freakin steep slope, and before I know wtf is going on I'm bouncing off the tops of them, already going closer to 35mph, completely unable to steer. It was at that point I realized I was seriously screwed and assumed the fetal position, hoping to at least avoid any mangling of limbs as I helplessly fell down the glacier. Fortunately I came to a rest at the base of one of the larger moguls, on my feet no less! I realized I had stopped, looked around and saw about five ski patrol guys just staring at me, looked down and saw everything was still attached, then looked up again and now there were about fifteen of them all around me, just staring like "wtf just happened" Somehow I wasn't even hurt at all, just very dazed and tired. It took me forever to collect myself and get down after that, but I did.

Cliffs: Skiing on an Austrian glacier, no visibility, nailed some moguls with no warning, tumbled, lots of freaked out ski patrol guys.

This is rather concerning, especially with the avatar you both have...

Sounds like you just got lucky no one had a crab pot handy. :D

This is why I use jack + jack stand(s) + wheel chocks

I think the most I've ever been scared was when I was about 22 and we moved into a new place. There was a mirror on the wall just outside the bedroom.
I got up in the middle of the night to pee. I was very groggy. I walked into the hallway and there was someone in front of me. I swung out as hard as I could to knock the fucker out. My swing missed, my heart pounded.
I had swung at my dimly lit reflection in the mirror. Luckily, I missed.

That was scarier than the axe murderer.

EDIT
Friggin spel check.

That was scarier than being attacked by the axe murderer at the rest stop?
